Register for Online Services
To register for online services please contact the surgery by phone, email ([email protected]) or via the contact form (link above) and ask to be set up for online services. Your registration document will then be produced by the practice staff and you need to come in and collect it. When you come and collect your registration document please bring with you some form of photo ID (drivers licence, passport, bus pass etc.) so that we can be sure we are giving the registration details to the correct person. Parents will be able to collect detail on behalf of children under 18. If you wish to collect the registration documents on behalf of another adult please bring with you written, signed authorisation from that person.
