Registering with Usstethoscope

Newly Registered Patients

In order to register with this Practice as a permanent patient, you must be resident in our practice area as shown below and for settled purposes (usually considered to be for at least three months)

Please navigate to Our Practice Area page – catchment area tool, where you can check if your post code is within our catchment area.

Process of Registration

  1. Download the Registration Information and complete
  2. Bring into the surgery during our opening hours
  3. Also bring along two forms of ID, ideally the following:
    • Passport or Driving Licence
    • One form of ID that shows your address, e.g. Bank statement, Utility Bill etc.

Temporary Residents

If you have friends or relatives staying with you and they become unwell during their stay, the Doctors are able to offer treatment. If they are staying in the area for less than 3 months they will be seen as a temporary resident and information on any treatment will be sent to their usual GP. If they are staying any longer than 3 months they will need to register as a permanent patient.

Patients from other countries

Patients from other countries can be seen for emergency treatment for urgent problems. However it is at the practice’s discretion whether to accept patients visiting from abroad for routine care.
